when someone says who's responsible for a certain violent outbreak,

can you blame it on one person? No outbreak of violence can happen at the behest of one person alone. Even if one person were to give the orders, he is able to carry the day because he enjoys the support of others in this exercise. Some may be more responsible (but that is a matter of degrees).
