Good manners boils down to making all present the most at ease one possibly can. I believe I would give her as many visual and audible god examples as possible. She will get it. Perhaps she did not get the training you and your other friend did. You will do her a favor by not calling it out, just showing by example, I think.

It depends. She might have felt silly having not thanked him before you...
but if it has happened before, or if it happens again, then I'd say something.

Is it a good friend, and will she be back?  If so, find a kind way to remind her, she just may have forgot...let's hope so.
