Oh, I’m so sorry! My mistake. All these years I thought that in Cyckoo’s Nest they were calling her Nurse Ratchet - because she”d enjoy tightening the screws on each patient! Hahahah!

I‘m sorry but I think the word for the nurse’s nickname is “ratchet.” It’s a tool and also is used in ghetto slang.
