Medics walked through the lines that people were waiting in to look for signs of illness if you were noticed a code was written in chalk on your back.  One mark designated a sickness and you went through the door on the left leading to the Hospital island next door.

If you had all of the items you mention, including your sponsors name, you went through the door on the right to Manhattan Island.

If you were well, but didn't have the paperwork on the sponsor, you went through the middle door, which meant you were headed out west to work on building the railroads.  Most going out west were from Ireland.

A short time later they eliminated the middle door, because of the influx of Chinese immigrants on the west coast.  They went east to where we were building the railroads.  Because their wages were a fraction of what was being paid to the Irish.

You are correct.  Have you ever been there?  I taken groups there more than 50 times and each time our special guide fills us in o how it worked.

I have seen on Ellis Island ships' manifests where certain immigrants, some of whom may have been related to me, were pulled aside and not allowed to enter (I don't know for how long, or if they were possibly being sent back) with a notation of three initials indicating they were a risk for being indigent, so this concept, if I understand it correctly, is not new to me. My ancestors had to have a certain amount a cash, a sponsor, and a destination with a person named at said place.
