A guy I'd been with for about a year, and then broke up with,
told me a month or so later, "I actually was considering offering you my family name."
At the time, my first reaction was much like yours. He assumed I would be kicking myself for the chance I'd missed.
I wasn't, and never have, though I do realize by now that he may have been inept, but he was sincere, and his family name was damned important to him. He was a lot younger than Patrick.
