Believe me, when these riders don't want to race, it is pretty serious.  My son has had a few concussions, it is nothing to take lightly.  Repeated injuries can make a phenomenal rider like Travis Pastrana for example into a crash and burn rider.  If Stewart doesn't watch out, he will go that route as well.

And yet, when you're actually there, it's like, "You know, this really isn't all I had hoped it would be."

So, they essentially get to decide for themselves whether they will still race despite injuries, concussions, etc.?  They don't need "medical clearance"?
