So, other than locking them up in an institution, or else returning them to family, do you propose any better solutions?

I am sorry for your struggle
I work as a mental health worker in a prison and I've worked with both men and women. what an extreme struggle. I don't know the ages of anyone involved in your journey but many times offenders that I work with will take their medications once they realize that they can get "cooler" stuff while on meds.
They have to get to the point where they do a cost benefit analysis and come up with the fact that life would be a heck of alot more profitable one meds and stable. the struggle with this is they miss the euphoria of mental illness (dependent on diagnosis).
finally, I agree with you concerning consequences. mental illness is not an excuse for maladaptive/criminal/inappropriate behavior. The person involved is responsible for developing stability. sort of like the person who knows they get completely stupid on whiskey. they must be help accountable when they start drinking.

Sad Story
and a common one. From what I have heard, most people on such medication do not want to take them because it somehow diminishes them.
