Your list seems to cover what I consider cheating . . . .
I always joke that after the "till death do us part" portion of our vows, my wife said to me, ". . . and if you don't behave, death will us part!"
One can also cheat emotionally, which is probably the easiest to detect; it is as if you are no longer with a person, they are "with" you, but really somwhere else. This happens a lot with spouses who may not have cheated physically with another, but yearn for that person, or maybe it is an "innocent" on-line flirtation that begins to take up all of a person's emotional energy, leaving none for the spouse.

If the ex happens to be the mother of his child/children, I hope you make an exception. Flirting with the ex would, of course, be different.
As for cheating...I'm going to go with a very conservative stance...when you think of your own happiness before your partners then you are cheating.
