I have been thinking what to say, and find that others have said just about all I've thought of, but this: The thought of you giving up the house where you and Wiley and Graysome have been so happy, where you have put so much of your heart into making it home, and not just for yourselves--it is as heartbreaking as putting Wiley's name on that list.
If there is not a care facility close to home, perhaps you could rent a small flat near it when the time comes, but keep the house for the occasional respite visit. Perhaps on his better days, you could bring Wiley home for an overnight or a weekend...? Maybe you can arrange with a neighbor/friend to look after the birds and critters?

Darling S. Sister, your post touched near to my heart, and if I were a poet I would tell you reading it rent my being deeply, slashing near to my soul, and I felt your pain. Little by little, the path to choose that's right for you and dear Wiley John will open, showing you the way.
Even if you DO agree to put Wiley on the dreaded list, perhaps there is no obligation to act when an opening occurs if you are not ready for that option? Hugs to you both.
