Have you tried discussing your situation with this landlady? I feel if she has a human bone in her body she might consider a key option for you. Perhaps a legel aid group could assist. From a legel point of view, here in California, there was a property owner/landlord who was sued because of so many violations at his rental units; he was found guilty and sentenced to, drum roll, a stint of living in his own units (complete with rats, roaches, assorted vermin, lack of proper heat and running water, etc.)! There's always hope, I hope.

first make sure that you are up to date on your rent and then ask for a key. I feel sorry for the tenant who is so ill as well. I mean where does he go now to use a bathroom.

Well you are not going to improve the relationship with the Landlady of you report her Bill. Couldn't you get another key cut to save disturbing Mrs S. Or failing that hold a little stock of nonperishable foodstuff for emergencies.
