Depends---does the carer make a habit of this kind of behavior? You didn't mention that part in the post. If that carer was just having a bad day and is fine the rest of the time, I'm afraid that you might have overstepped. If, however, that carer makes this kind of behavior toward clients a habit, then yes, you have to say something for the sake of those who cannot speak for themselves.
