There is a rule in place, twelve seconds when no one is on base, but it is never enforced. I do believe that with pace of play becoming a big issue, it will get enforced.

I don't think so, although perhaps they should. However, the scenarios change constantly throughout the game. If there's no one on base, the pitcher shouldn't take too long between pitches. But once runners get on, the thought-process changes, and the pitcher suddenly has more to think and worry about. It's understandable that he slows down a bit. Putting a clock on them probably wouldn't help matters much . . .
